Documentation Guidelines

To ensure our support is being directed solely to the population we are committed to serving, we request paperwork from caretakers showing legal documentation for the child(ren) in their care.

Contact Information Needed

The following items must be current for Primary Caregivers.

  1.  Address
  2.  Phone Number
  3.  Email Address
  4.  Government-issued Photo ID (Driver’s License, ID Card, Passport, Military ID, etc).
  5.  Facebook Identifier (only if you plan to access our Facebook Marketplace page)
  6.  Signed Family Terms and Conditions
  7.  Foster License (if applicable)
  8.  Name Change Documentation (if applicable)

Accepted Documents

In order to shop in our Resource Center or participate in our programs, we need to have at least one of the below Accepted Documents on file for all Primary Caregivers/Children.  The good news? You likely already have this required paperwork on hand or should be able to easily attain it through your caseworker. 

    ACCEPTED DOCUMENTS

  • Notice to Provider (NTP) signed and dated within
    the last 6 months
  • Adoption Decree
  • Temporary Guardianship signed and dated within
    the last 30 days
  • Permanent Guardianship Decree
  • Letter of Guardianship and/or Annual Report dated within
    the last year
  • Tribal Placement Paperwork
  • Interstate Compact for the Placement of Children (ICPC)
    and the contact information for your AZ caseworker
  • Extended Foster Care Agreement 

NO LONGER ACCEPTED

  • Power of Attorney
  • 3rd Party Decision Making Letter
  • In Loco Parentis
  • Danger Plan / Emergency Placement
  • Notice of Hearing for Placement, Adoption, or Guardianship
